Scheduling Appointments
We take pride in being on time, and will see you promptly. We ask that you be on time as well. If you have to reschedule your visit, please do so 48 hours in advance. This courtesy will allow another patient to be seen in your place.
Please call 480-831-0700 to schedule your appointment as soon as possible. Same day urgent appointments are available.
Most foot and ankle office procedures can be done at our office, and can usually be performed within your appointment time. X rays, orthotics, wound care, and treatments for ingrown nails, fungus, warts, biopsies, etc. are simple and painless and allow quick return to the day.
What to Bring to Your Appointment
If you are a first-time patient, please plan on arriving early to complete a New Patient Registration Form and New Patient History Form. You can also click on the hyperlink to these forms to print and complete them prior to your appointment. Please bring the following items to the appointment.

Referral (if required)
•
Valid identification and all insurance card(s)
•
Pertinent medical and surgical history information and a list of all prescription and over-the-counter medications as well as any allergies you may have

X-ray films or diagnostic information, orthotics, or other shoes you would like the physician to evaluate
Insurance and Payment Information
We are a provider for most major insurance plans. However, we encourage all patients to confirm their coverage prior to making an appointment. Co-payments and insurance deductibles are due at the time of the appointment. Please call us with any questions regarding which insurance plans we accept or other billing concerns. Insurance coverage and benefit questions should be directed to your health insurance provider.
